SUBJECT:     Request for Comments – Benefit Appeals Report, ETA Form 5130;  OMB Number 1205-0172

PROGRAM(S):         UI

1.  Purpose.  To transmit a request for public comment on extension of an existing Information Collection Request (ICR), which will be submitted to the Office of Management and Budget for review and approval, concerning benefit appeals reporting on ETA Form 5130 under the Federal-State unemployment compensation system.
